Please deliver the necessary documents, listed below, in order for us to order the permit from MVC.

3D Driving school needs the following:

1. MVC Learners Permit form (link below to print). Fill out student information and be sure to have both the student and guardian sign. Leave blank the Driver License Number and any section relating to 3D Driving School. Please write out the eye color in full.An Original birth certificate (not a copy) or a valid passport.
2. A check for $10 made out to MVC.
3. Proof of written test. If the student has not completed the written test please note. We'll need to schedule a test time/date once the permit is in hand.
4. A copy (not the actual card) of the student's Social Security Card.

Once 3D Driving School receives the permit your assigned instructor will call to get the lessons scheduled. 

Please drop the documents (in an envelope) in our lockbox located by the front door of the office at 283 Sparta Avenue in Sparta.
